Severe obesity is associated with multiple comorbidities which reduce the life expectancy and markedly impair the quality of life.Obesity-related problems begin at the head and end at the toes, affecting almost EVERY ORGAN in between.


Obesity and Diabetes
Obesity is a frequent etiologic factor in the development of type 2 adult onset diabetes mellitus. Morbidly obese patients can be very resistant to insulin due to the marked down-regulation of insulin receptors.
The tendency toward hyperglycemia manifested by obese patients is another risk factor for coronary artery disease as well as for fatal subcutaneous infections.
Gastric-surgery-induced (gastric bypass) weight loss performed both open and laparoscopically, is associated with resolution of the diabetes in 85% of patients and this effect is long-lasting . It has been found that the earlier the patients with type 2 diabetes mellitus undergo weight reduction gastric bypass surgery, the greater is their likelihood to have complete resolution of their diabetes.

In another study, it was noted that patients without diabetes or hypertension are significantly younger by 5 years than those with either diabetes or hypertension and are significantly younger by another 5 years than those with both diabetes and hypertension.
Furthermore, patients were more likely to correct their diabetes and hypertension, the more weight they lost.
These data support earlier operations in morbidly obese diabetic patients before their diabetic related complications develop (neuropathy, retinopathy, and renal insufficiency).

Peripheral insulin resistance, primarily in skeletal muscle, is present in morbidly obese patients and may resolve with surgically induced weight loss.

Is gastric bypass helpful??

There are data suggesting that type 2 diabetes mellitus could be a disease of the foregut.
The gastric bypass operation has been found to decrease the progression and mortality of non-insulin dependent diabetes mellitus when compared to a matched control group of patients who either chose not to have surgery or were unable to obtain insurance coverage.
So it seemed wisely if you are thinking of surgery and You are diabetic ,think primarily of gastric bypass . Gastric bypass is most preferred in your case of severe obesity .
