What exactly do I need to do to get the process started to get the lap-band surgery? I have Medicare and Medicaid, so it will be covered, I just have no idea where to start.

Do I call my doctor's office and make an appt. saying 'I'm fat and I want surgery!" ??

I'd love to hear from anyone who has been or is going through the process so I can figure out how to START. Do you have to be on some kind of weight loss program thru the dr. already?
